Calcitonin:
Regulates plasma calcium by inhibiting bone resorption used in pts in whom hormonal therapy is contraindicated.
Salmon calcitonin 200 iu daily should be used with cal and vit D.
Fluoride:
Potent stimulator of bone formation.
Slow release sodium fluoride 25mg b.d. given 12 of every 14 months combined with cal adviced.
More than 4 yrs adm can lead to toxic accumulation of fluoride in bone so it is reserved for established postmenopausal osteoporosis.
Tibolone:
It is structurally related to the 19 nortestosterone progestins used clinically in oral contraceptives. 1.25mg /day is acceptable for prevention of bone loss, 2.5 mg is more effective for alleviation of hot flushes.
Prevents bone loss ass with GnRH agonist treatment.
The beneficial effect is attributed to the estrogenic metabolites.
Teriparatide:
Is the recombinant human 1-34 amino acid fragment of PTH.
Given in postmenopausal women with osteoporosis, once daily subcutaneous dose.
As expensive and difficult in administration used in individuals with serious osteoporosis at high risk of fractures.

Irrespective of receiving daily oral or future injectable depot therapies, these require health care visits for medication and monitoring of safety and response. If patients are treated early enough, before a lot of immune system damage has occurred, life expectancy is close to normal, as long as they remain on successful treatment. However, when patients stop therapy, virus rebounds to high levels in most patients, sometimes associated with severe illness because i have gone through this and even an increased risk of death.
